Thank you for making an appointment with us. We appreciate your trust and confidence in us. In order to better serve you, we ask that you come to the appointment with the following items:

If you have made an appointment and created a profile on our patient portal (9460.portal.athenahealth.com), you could print and fill a New Patient Packet and send with a copy of your Driver’s License or ID and your insurance card to info@pima1.com.

Also, we ask that you please call your insurance company to determine whether or not we are listed as providers. Remember it is the patient’s responsibility to know if his/her insurance requires a referral. Please call our office prior to the appointment to verify if a referral has been received. Failure to do these things could lead to your appointment being rescheduled.
